Log In
Everyone sees the same “Welcome” screen upon logging in, including first-timers, and you may notice a navigation menu on the side of your screen with more Microsoft 365 productivity apps to help you switch between applications. Your Microsoft Forms dashboard will already be loaded as your My Forms section.
Create a Form and Choose a Form Name
Click on the (+) plus sign to create your first new Form, and the widget will prompt you to name your Form and add a Form description before moving into the Questions section.
Choose Your Form Questions
After adding your Form name and description, click on the button that says “+ Add New” to start adding questions to your Form. Your question style options will include:

Preview and Test Your Form
Before you click “Send” to share your Form and begin collecting responses, this is the point when you can preview your Form to review what you’ve entered and be certain your Form is ready. In Preview, you can also see how your Form will look for mobile users.
You can also test your Form in Preview mode by logging test responses. If you need to edit your Form at this point, you can click “Back” and make changes. If you are finished with your Form and you’re ready, click “Send” and a menu will give you options for how to share your Form, including embed codes and email.
